The Green River Regional Educational Cooperative is pleased to offer Student-Focused Transition Planning. Student-Focused Transition Planning is a process-oriented approach to empowering students with disabilities. Student-Focused Transition Planning involves the development of methods and resources to guide facilitators in intensive post-secondary transition planning to assist individuals in identifying and pursuing their goals and dreams.
In order to best implement this process, a team approach is recommended.
- Participants will apply a procedure to ensure students’ goals and dreams are at the center of transition planning.
- Participants will identify students on caseloads who may need a more intensive level of transition planning.
- Participants will develop an action plan for implementation and sharing of Student-Focused Transition Planning procedures and strategies with other special education teachers in their district.
